Care transition
Care transition is a link between stationary and ambulant care. Its goal is to organise an ambulant care of the patient after his stationary stay in such a way that his needs are met in the home setting as well.
Right after the patient’s admission to the hospital, out care transition would like to recognise the problems regarding nursing care after his discharge early enough and discuss them with patients and his relatives promptly in order to organise proper help. This includes nursing care and domestic help through a social station, ordering lunch as well as shopping for medications, medical aides. The relatives remain involved, they get relevant instructions about domestic help that the patient will need.
The most seriously ill patients in their last life phase should be
given an opportunity to spend time in their home setting once again. It
is a matter of particular concern for care transition to offer these
patients a good life quality in the familiar setting.
